Tomoya Okazaki encounters Nagisa Furukawa, a girl who speaks to herself, on his way to school. During lunch, he learns she is repeating her final year of high school and hopes to revive the defunct drama club. Later, he visits her family's bakery, meets her parents, and is invited to dinner.

Nagisa expresses her desire to revive the drama club, and Tomoya assists her by designing promotional posters placed around the school. During the day, Tomoya meets two new girls—Kotomi Ichinose, an eccentric genius, and first-year student Fuko Ibuki. Nagisa later discovers Tomoya's past as a basketball player and invites him to play, but it is revealed that an injury from a fight with his father has left him unable to play.

Tomoya checks on Nagisa after a recent incident and learns of her lifelong physical frailty. On his way home, he helps an unusual electrician named Yusuke Yoshino, later revealed to have been a professional musician. The next day, Tomoya assists Nagisa in building confidence for the drama club by role-playing as a new club member.

Tomoya encounters Fuko at school, where she invites him to her older sister's wedding, but he declines. With Youhei's help—bribed with bread from Nagisa's family bakery—Tomoya attempts to revive the drama club, while Nagisa seeks assistance from Ryou and Kyou. Tomoya also tries recruiting Tomoyo and Kotomi, with mixed results, and later learns from Nagisa that Fuko is hospitalized.

Tomoya and Nagisa invite Kouko, Fuko's older sister, to the Founder's Festival, while Fuko distributes carved starfish and her fan club promotes Kouko's upcoming wedding. Nagisa and Tomoya encounter Tomoyo, who is in a bear costume and helps maintain order by confronting troublemakers. When Kouko arrives, she is unable to see or hear her sister.

Tomoya and Nagisa persuade Kouko that Fuko would want her to marry, despite Fuko's comatose state. They approach the school administration after learning Kouko wants to hold the wedding on campus. Meanwhile, Kouko reveals Fuko's condition has worsened, and Youhei notes that some students can no longer see Fuko or remember her gifts.

As more people from school lose the ability to see Fuko, even those who were once close to her begin to forget her, including Youhei, Kyou, and Ryou. Tomoyo is able to remember her with Tomoya's help, and Youhei briefly recovers a memory before losing it again. Tomoya and Nagisa throw Fuko a birthday party to cheer her up, but upon returning to the bakery, they find that Nagisa's father has forgotten her and her mother can no longer see her, prompting Tomoya and Nagisa to take Fuko back to school.

Tomoya, Nagisa, and Fuko spend the night at school celebrating Youko's upcoming wedding, but in the morning, Tomoya and Nagisa initially forget about Fuko and cannot see her. After remembering the wedding, they regain sight of her. On the wedding day, only the three of them arrive at first, but later many others who received a starfish also show up. After expressing gratitude and congratulating her sister, Fuko disappears, leaving behind a lingering rumor of a mysterious girl running through the school.

Tomoya encourages Kotomi to join the drama club and helps her connect with others at school. Later, he brings her and others to the drama room, where he invites them to join. While Kyou initially declines, she eventually agrees after encouragement from her sister, and Kotomi also joins, though she leaves briefly to listen to a violin being played.

Kotomi struggles to improve her violin playing, and Kyou arranges for her to perform at a recital, hoping practice will help. On the eve of the recital, Tomoya visits Kotomi, eats her homemade apple pie, and has a strange dream. Despite the effort, Kotomi's performance is as poor as before, and afterward, she is approached by a mysterious man who frightens her.

Kotomi returns a violin to Rie Nishina and begins spending time with her, altering her routine to attend her classes. Inspired by Kyou's suggestion, Kotomi, Ryou, Nagisa, Tomoya, and Kyou spend a day together in town, where they encounter Fuko, though none of them recognize her. Later, Nagisa panics over a false report of Ryou's bus accident, and although no one is harmed, Kotomi has an emotional breakdown and disappears, leading Tomoya and others to investigate her whereabouts.

Tomoya discovers that he had met Kotomi as a child, though only she had remembered their past friendship. During that time, Kotomi's parents left for business on her birthday and were later killed in a plane crash, leading her to destroy her father's important work. As Kotomi isolates herself, Tomoya begins clearing the overgrown garden of her home, later joined by Nagisa, Kyou, and Ryou.

Kotomi discovers Tomoya has spent the night working in the garden and fallen asleep. At school, she is surprised to find Tomoya, Kyou, Ryou, Nagisa, and the mysterious stranger waiting for her. The stranger reveals himself as her legal guardian, clarifying that there were no written notes for her parents' thesis and that the envelope Kotomi had burned was insignificant. He also gives her a birthday gift: her parents' suitcase, recovered after years and containing a teddy bear and a heartfelt letter. Moved by the gift, Kotomi finally begins to heal from her grief.

Nagisa attempts to revive the drama club by gathering members and seeking an advisor, leading her to Toshio Koumura, who is already assigned to the choir club. A conflict arises when Nagisa consults Rie Nishina about the situation, and a threatening letter appears in Nagisa's desk, traced back to Rie's associate Sugisaka. After learning about Rie's past, Nagisa decides to abandon the drama club, disappointing Youhei, who later tries to convince Tomoya and Nagisa to participate in a basketball match to pressure the choir club, but Tomoya refuses.

Mei visits her brother Youhei and temporarily stays with Nagisa while in town, as she cannot stay at the all-boys' dorm. Tomoya and Youhei enlist Kyou's help for a basketball game, which starts with a strong lead against first-year players before the opposing team's starters even the score. Tomoya makes a final shot to win the game, after which Mei returns to her hometown.

Following a basketball game, the choir club proposes sharing Koumura with the drama club, but the student council refuses. Nagisa falls ill and misses school, prompting Tomoyo to ensure Tomoya and Youhei arrive on time. Meanwhile, Kyou continues arranging time for Tomoya and Ryou, and Tomoya uses a charm to spend time alone with Kyou, which leads to an unexpected situation. After resolving the charm, Tomoya and Tomoyo face a group of gang members at the school entrance.

While Tomoya is suspended from school, Tomoyo visits daily, helping him with meals and gently inquiring about his family. On a Sunday, several friends bring food to Tomoya's house, including an unexpected visit from Fuko. To help Tomoyo's reputation, which is being damaged by rumors, Tomoya arranges for her to compete against school sports clubs. Because of the tension at home, Tomoya stays with Nagisa, where he discovers Sanae running a cram school and Nagisa struggling with repressed memories of a past mistake.

Tomoya learns about a mysterious story Nagisa wants to include in her play and inquires with her parents, who are unaware of it. Akio discovers Tomoya and Nagisa in a shed and later reveals a painful past incident in which Nagisa nearly died after being left alone as a child, prompting her parents to abandon their careers to care for her. During a family picnic, Nagisa unintentionally expresses her feelings for Tomoya during a game with local boys.

Nagisa and her friends prepare for their school play by working on various aspects such as dialogue, music, lighting, and staging. Meanwhile, Nagisa's father rents a video of a play that holds special meaning for her. Mei becomes concerned about her older brother Youhei's lack of direction after high school and temporarily moves in with Nagisa. To ease Mei's worries, Tomoya suggests Youhei get a fake girlfriend, but several attempts to find one fail until Sanae offers to help, unaware Youhei still believes she is Nagisa's older sister. When Mei meets Youhei's "girlfriend," Sanae pretends to be a student at Youhei's school.

Yohei and Sanae go on a date with Mei, Tomoya, and Nagisa, where they witness a young girl being bullied. Her older brother intervenes to protect her, prompting Mei to reflect on how Yohei used to be protective before joining the football club. The group attempts to provoke a reaction from Yohei by pretending to pursue Mei romantically, but he remains unresponsive, leading Mei to decide to stay and address the situation.

Youhei has been absent from school, causing concern among his friends, including Mei. Mei, Tomoya, and Nagisa visit the soccer club to persuade them to let Youhei rejoin, but their efforts are met with resistance. Tensions escalate when the soccer club leader mistreats Mei, prompting Youhei and Tomoya to confront the team, leading to a physical fight and a subsequent emotional resolution between Youhei and Tomoya over Mei's well-being.

Tomoya and Nagisa visit Youhei and spend time with Misae and Tomoyo. During a private conversation, Tomoya falls asleep, and the episode recounts Misae's past encounter with Katsuki Shima, a mysterious boy who made her a promise in exchange for her kindness. Misae develops feelings for Igarashi, only to discover through Katsuki's indirect actions that Igarashi is already in a relationship, leading to her emotional departure.

Misae moves on from Igarashi and grows closer to Katsuki, eventually falling in love with him. Katsuki later discovers he is not himself but a cat sent by the dying real Katsuki to fulfill Misae's wish. Tomoya, Nagisa, and Youhei learn that Yukine is respected by two rival gangs for helping injured members from both sides. Meanwhile, a boy named Yu seeks Yukine for help finding his older sister, who is dating someone from her brother's gang. As the group uncovers more about Yukine's family and the gangs, tensions rise when members of the rival gang mistake Youhei for Yukine's brother and attempt to confront him.

Tomoyo warns Yukine and the others that the police may intervene if the gang conflict escalates further. To prevent violence, Yukine and Tomoya attempt to negotiate with the rival gang, leading to a challenge between Yukine's presumed-dead brother Kazuto and the opposing leader. As tensions rise, Sanae's enchanted bread incapacitates most of the group, leaving Tomoya to fight the rival gang's leader while Yukine impersonates Kazuto to end the conflict. She later reveals that Kazuto has been dead and wished for peace. At his grave, Yukine is joined by the rival gang in mourning, and Tomoya witnesses a symbolic glow as peace returns to the town.

As Tomoya and his friends approach graduation, they begin to plan their futures, each with different aspirations. Nagisa, however, is unable to attend school or take exams due to a recurrence of her illness. On graduation day, Tomoya returns home to Nagisa, who asks to take a walk with him, holding hands.

Tomoya, after graduating, takes a job at a bakery and later moves out of Nagisa's house to live in a small apartment, working as an electrician under Yusuke Yoshino. Nagisa begins a new school year but struggles to make friends and revive the theater club. After a tiring first day at work, Tomoya is too exhausted to stay awake during dinner with Nagisa.

Tomoya struggles to balance his new job with his personal life, particularly his relationship with Nagisa, as he tries to hide an injured shoulder from his boss and keep a promise to attend the Founder's Festival with her. While his dedication earns him respect at work, his growing workload causes him to become increasingly absent from his personal commitments.

Yusuke shares stories of his past with Tomoya, including how he met Kouko in high school, his rise as a singer-songwriter, and his eventual downfall into drug use. Tomoya faces personal challenges when his father's legal troubles disrupt a new job opportunity, causing him frustration and anger. Meanwhile, Tomoya proposes to Nagisa, and she happily accepts.

Tomoya attempts to persuade Akio to discuss his marriage to Nagisa, but Akio agrees only if Tomoya can beat him in a baseball match. After improving his skills and hitting a home run, Tomoya pleads with Akio, who eventually consents. The episode also covers Nagisa's 20th birthday, a brief misunderstanding due to her drinking and illness, and her eventual graduation, followed by her marriage to Tomoya.

Nagisa and Tomoya, now married, face new challenges as Nagisa begins working at a family restaurant with Nishina. With the help of midwife Yagi, introduced by Sanae, Nagisa prepares for the delivery, though Sanae warns Tomoya of the risks involved due to her fragile health. Despite the dangers, Nagisa decides to continue with the pregnancy, and she and Tomoya choose the name Ushio for their child after a quiet walk by the river.

As winter approaches, Nagisa falls ill again during the final months of her pregnancy. On a snowy day, she goes into early labor, and unable to reach a hospital in time, her family must deliver the baby at home. After a difficult birth, Nagisa gives birth to their daughter, Ushio, but soon after, she dies from complications related to her illness and the delivery, leaving Tomoya devastated and his hopes for a family future in ruins.

Five years after leaving Ushio with Sanae and Akio, Tomoya reconnects with Sanae, who invites him on a family trip with Akio. When Sanae and Akio unexpectedly leave Tomoya and Ushio alone, the two spend time together, and Tomoya eventually invites Ushio to continue the trip with him, uncertain of where they will go.

Tomoya buys a toy robot for Ushio during their trip, intending for it to entertain him in the countryside. The next day, while exploring a flower field, Ushio loses the robot, prompting Tomoya to search for it. Recalling a memory from his past, Tomoya travels to another location where he reunites with his grandmother, Shino Okazaki.

Tomoya and Ushio return from their trip and begin living together in Tomoya's apartment after staying with Nagisa's parents. Tomoya takes a day off to meet with Kouko and later visits his father, successfully persuading him to move back to the countryside with his mother. As they bid farewell, a mysterious light is absorbed by Tomoya, noticed only by Ushio.

Tomoya brings Ushio to school, where he meets her teacher, Kyou Fujibayashi. At work, Yoshino asks Tomoya to let Fuko spend time with Ushio, and later, Tomoya uses Nagisa's lullaby to help Ushio sleep. Kyou invites Tomoya to participate in the school's sports day, which he initially refuses but agrees to after Ushio's encouragement. Later, Ushio is seen walking alone near the new hospital, where she meets Fuko. While preparing for the event with Sanae and Akio, Ushio unexpectedly falls ill with a fever.

Tomoya stays home to care for Ushio, whose fever persists, and enlists Akio and Sanae to assist. As winter sets in and both Tomoya and Ushio grow weaker, Ushio's repeated request for a trip eventually leads Tomoya to agree. After leaving their home, Ushio's condition worsens, and she dies in Tomoya's arms, expressing her love for him before he collapses as well.

In the Illusionary World, a girl near death in the snow reveals to a remorseful robot that they once lived in another reality, singing a familiar tune as her world begins to collapse. Meanwhile, in the real world, Tomoya and Nagisa break free from their tragic fate—Nagisa survives Ushio's birth, and together they raise their daughter in happiness as lights fill the sky.
