On a quiet evening, the world is thrown into chaos with the arrival of alien parasytes that seek to consume human brains. A parasyte egg lands at the home of high school student Izumi Shinichi, and when it hatches, the creature attempts to take control of him. However, Shinichi resists the invasion, preventing the parasyte from reaching his brain.

Shinichi and the parasyte in his right arm, who introduces himself as Migi, continue their uneasy partnership as Shinichi uncovers a link between parasytes and a series of murders. Struggling with his isolation and frustration, Shinichi is soon to encounter another parasyte for the first time.

As the mincemeat murders continue, Shinichi struggles with the burden of his secret knowledge. When Migi detects another parasyte, Shinichi is shocked to learn it is at his school. He grows increasingly suspicious during a school assembly where a new, attractive teacher is introduced.

After the recent turmoil caused by A, Shinichi's life appears to return to normal, though he struggles with lingering sadness. His mother, Nobuko, grows concerned when his attempts to hide his feelings fail, while his father, Kazuyuki, hints that a childhood event may be the root of Shinichi's distress. Nobuko recalls a painful memory involving a scar on her hand from when Shinichi was young.

Shinichi rushes to school and encounters Nagai being harassed by a group of delinquents. He intervenes and meets Kimishima Kana, a high school student. As Satomi tends to Shinichi's injuries, a phone call from his father sets the stage for a dramatic shift in his life.

Shinichi narrowly escapes death after being attacked by his mother, who has been transformed into a dangerous parasite, thanks to the intervention of Migi. Upon waking, he is forced to confront the painful truth that the horrifying events were all too real.

Believing he has found the person responsible for his mother's death, Shinichi encounters Uda Mamoru, a man with a similar personal vendetta. Uda, understanding Shinichi's pain, proposes a partnership to achieve their shared goals. With the moment of reckoning at hand, Shinichi prepares to confront those responsible.

Shinichi begins to adjust to a more normal life, enhanced by Migi's cells that have significantly boosted his physical capabilities beyond even Mitsuo's level. He, however, grows concerned that these changes may affect his mind as well. As things settle at his school, a new and unexpected parasite emerges.

Yuko becomes intrigued by Shimada Hideo, but begins to suspect something is wrong with his appearance. As she investigates further, she uncovers a link between Shimada and her brother's ongoing police case.

Shimada Hideo, driven to a feral state, goes on a violent rampage, targeting Satomi and her classmates. Amid the chaos, Shinichi navigates the destruction in a desperate attempt to rescue Satomi.

Kana's growing affection for Shinichi contrasts with his increasing concern over her parasitic abilities. As normalcy resumes, Satomi becomes uneasy about Shinichi's behavior, while a new parasite emerges, causing chaos in a local gang's territory.

Kana's unique power continues to grow, and concerned for her safety, Shinichi reveals the truth about the parasites, hoping to protect her. However, Kana becomes suspicious and believes Shinichi is deceiving her, leading to a growing emotional distance between them.

Following Kana's death, Shinichi appears to have returned to his usual routine, though his emotional detachment worries those around him, including Satomi and her friends. Unbeknownst to them, Shinichi is troubled by his own unsettling mental state. At the same time, a mysterious man starts investigating him.

Shinichi's secret is discovered by Kuramori, a private investigator working for Tamura. When Shinichi attempts to tell Satomi, the conversation ends in misunderstanding and creates tension between them. To resolve the situation, Shinichi turns to Uda for assistance.

Kuramori, still uncertain, tentatively accepts Shinichi as an ally. His resolve is tested when he witnesses a parasite kill a human, causing him to doubt their mission. As Shinichi grows frustrated, a new parasite emerges, disrupting their fragile truce.

A new parasite, unlike the others, appears with a friendly and talkative personality, introducing himself as Miki. As Miki quickly takes action, Shinichi is forced to flee while Migi remains unconscious, leaving him with no choice but to keep running for survival.

Kusano's group targets Tamura for elimination after identifying her as a threat, while Kuramori, seeking revenge for his family's murder, kidnaps Tamura's child. A tense confrontation ensues as Kuramori, holding the baby, forces Tamura to a dangerous cliff and issues a deadly ultimatum.

Shinichi and Migi encounter Tamura at a park, where she is holding her child. Shinichi is visibly upset by the situation, while Tamura confronts them with intense questions about the parasites' purpose and their relationship to humanity.

Shinichi is brought before Uragami, a convict with an unusual talent, as Hirama investigates his possible link to a series of parasite cases. After being cleared and resuming his normal high school life, Shinichi is soon confronted again by Hirama, who reveals a government initiative led by Hirokawa to eliminate the parasites.

A coordinated effort between the police and a specialized strike force begins to eradicate a group of parasites led by Hirokawa, with Yamagishi's team deploying advanced sensor technology to track them down. As the operation progresses and human forces appear to gain the upper hand, an unexpected twist involving a single parasite threatens to shift the balance of power.

After successfully killing Hirokawa, the leader of the parasites, the assault squad faces a new threat with the emergence of the ultimate parasite. Meanwhile, Shinichi is troubled by the team's failure to eliminate Gotou, and he begins to wonder who will help him find the strength to confront him.

Shinichi, inspired by Satomi, confronts Gotou with Migi's help in a remote forest. Despite their efforts, they fail to defeat Gotou at a crucial moment, leading to Shinichi losing Migi and retreating in failure.

Shinichi takes a bold and risky action when faced with a seemingly impossible situation.

After defeating Gotou, Shinichi begins to return to a normal life, though he remains troubled by Migi's unexpected disappearance. As he tries to move on, a new and dangerous threat emerges.
