Sotobamura is a small village with approximately 1,300 residents, so remote that it is not connected to a single highway. It is an isolated community where old customs, such as traditional burial practices, are still observed. One day, the bodies of three people are discovered dead. Although Ozaki Toshio, the village's only doctor, feels uncertain, he initially treats the deaths as a normal occurrence. However, in the following days, the villagers begin to die one after another.

Meanwhile, Dr. Ozaki, after a strange meeting with Natsuno, begins to suspect vampires are behind the village's unexplained deaths and decides to monitor his newest patient to test his theory.

    Seishin discovers his father, who had been bedridden, has vanished mysteriously. Kaori arrives at the temple, disheveled and frantic, pleading with Seishin to grant her last rites in preparation for Megumi's impending attack, which she believes will claim her life along with the rest of her family.

    Natsuno is visited by Tatsumi, who reveals he is one of only four Shiki who can remain in sunlight without transforming. Meanwhile, Tooru discovers Ritsuko, a nurse, has turned into a Shiki but refuses to drink blood. At the same time, Dr. Ozaki manipulates Chizuru into attending a town festival, using her vanity to further his plan to destroy the Shiki.

    The village begins to take Dr. Ozaki's warnings about the Shiki seriously and starts planning their eradication. Meanwhile, Sunako, upon learning of Chizuro's death, instructs Tatsumi to ready the Shiki to either kill or turn the remaining villagers.

    As the first day ends, the village has killed many Shiki, but Dr. Ozaki suspects most have escaped. Tooru faces a difficult choice regarding Ritsuko's refusal to drink blood, and as the second day begins, the Shiki reveal a new method of attack.
