Duncan McLeod is drawn back into the Immortal conflict when challenged by the villainous Slan Quince. Connor MacLeod, an old friend and fellow Immortal, urges Duncan to reenter the battle between good and evil. Meanwhile, Duncan meets and befriends Richie Ryan, a street-smart teenager he discovers breaking into his antique shop.

Richie is deceived by a con man who pretends to be his father in order to exploit him, while MacLeod experiences a vision of his past, recalling the moment he first became immortal and was exiled by his own father.

Richie's friend dies from a lethal herbal drug, leading MacLeod to suspect it was created by the Immortal Kiem Sun, who aims to refine it for humanitarian use. However, a rogue disciple exploits the drug for personal profit, prompting MacLeod and Kiem Sun to pursue him. After discovering Kiem Sun's intent to continue his experiments, MacLeod ensures the drug's destruction.

An immortal friend of MacLeod's is killed in a remote mountain hideaway, where he had sought peace away from the ongoing war between immortals. A mentally impaired homeless veteran is accused of the murder, and the case is linked to the earlier death of Slan Quince, an immortal foe MacLeod had defeated. MacLeod works to prove the man's innocence and uncover the true identity of his friend's killer, while flashbacks reveal glimpses of his past during the American Civil War.

Felicia Martins, a young woman who appears to be a naive newcomer to the world of Immortals, seeks out Duncan MacLeod as her mentor and forms a relationship with Richie Ryan. As MacLeod becomes increasingly aware of her cunning and mysterious past, he realizes she is an experienced Immortal with a hidden agenda—to capture him and claim his powerful Quickening.

MacLeod, Tessa, and Richie are taken hostage at a courthouse by armed men seeking to free their imprisoned leader, Slade. When Slade's demands are refused, he selects a hostage for sacrifice, and MacLeod volunteers. After dying and resurrecting, MacLeod pursues the hostage-takers while the police remain unable to intervene.

Tessa is captured by a group of Survivalists living in the wilderness of the Pacific Northwest, forcing MacLeod to use his centuries-old skills to navigate the rugged terrain and rescue her. The group's leader, Caleb—an Immortal who has developed deep feelings for Tessa—faces off against MacLeod in a deadly duel between axe and sword in the remote wilderness.

MacLeod is critically injured in a car accident and taken to the hospital, where he is kidnapped by a disturbed physician conducting unethical experiments. After MacLeod escapes, the doctor kills a witness and plants evidence to implicate MacLeod in the murder.

Richie Ryan attempts to shield his former lover, Nikki, from a group of drug dealers, drawing the attention of the Immortal Voshin, a foe from MacLeod's past during World War II. Nikki's plan to use stolen drug money to secure a better future for her daughter adds tension to the situation, while Tessa's growing affection for the child introduces new complications in her relationship with MacLeod.

Rebecca Lord, believing her lover Walter Reinhardt has been killed by MacLeod, seeks vengeance and attempts to kill him with Reinhardt's sword. Unaware of the Immortal conflict, she engages in a personal battle with MacLeod, not realizing that Reinhardt is still alive and manipulating events to lure MacLeod into a final showdown.

A serial killer known as "The Scalper" targets blonde women, including a friend of Tessa's, with a pattern reminiscent of an Immortal MacLeod killed in 1925 after the man was betrayed by his lover. The modern-day killer is replicating the old murders in the same locations, prompting MacLeod and Tessa to investigate and stop the cycle of violence.

Tessa becomes a witness to the murder of painter Anne Wheeler, but without a body, her account is largely dismissed except by MacLeod, who helps her seek justice. As the investigation unfolds, it is revealed that Anne was killed by her Immortal ex-lover to conceal his secret. MacLeod discovers that the Chief of Detectives is the killer and works to rescue Tessa and expose the truth.

MacLeod is warned by his old friend Darius, a 2000-year-old monk, that Grayson, a former student and protege, is targeting Darius' mortal pupils in an attempt to lure the monk into battle. Grayson, resentful of Darius' shift toward peace, seeks to kill MacLeod and others, including peace activist Victor Paulus. MacLeod works to protect Paulus and later joins Richie and Tessa in France as Tessa begins a new job as a curator for a traveling art collection.

Duncan MacLeod becomes entangled in a case involving a mysterious mime who is actually Kuyler, an ancient and deadly Immortal who has operated as an assassin throughout history. French detective LeBrun, unaware of the supernatural stakes, suspects MacLeod due to his connection to a past murder. After a violent confrontation, MacLeod defeats Kuyler, ending his long legacy of violence.

MacLeod tracks down the ruthless Immortal Xavier St. Cloud, who has evaded justice for centuries through murder and theft, including an attempt on MacLeod's life during World War I. In modern-day Paris, Xavier uses poison gas to commit another crime, forcing MacLeod into a deadly confrontation. Meanwhile, Richie becomes entangled with an attractive but married woman, requiring Tessa's intervention to set things right.

Duncan MacLeod is drawn into a mystery at the Paris Opera when a strange death reunites him with Ursa, a massive, near-bestial Immortal he once saved from persecution. Now living beneath the city, Ursa has formed an attachment to an aging opera singer who manipulates him to eliminate her rival.

At a circus, Duncan MacLeod encounters Amanda, an Immortal with a complicated past, who is now being hunted by her ex-partner Blaine, also an Immortal. As Blaine escapes prison and seeks revenge, Amanda manipulates both men into a dangerous scheme involving a high-stakes heist.

Richie befriends a charming young woman who introduces him to the glamorous world of high fashion. His growing jealousy, however, leads to conflict with a renowned fashion designer who is also a powerful and deadly Immortal.

After surviving a fatal stabbing, Alfred Cahill believes he is divinely ordained to purge Paris of vice, leading him on a violent campaign against sex workers and their patrons. His crusade puts Tessa's old friend, now a call girl, in danger. Despite MacLeod's attempts to explain the nature of Immortality, Cahill remains unreachable, ultimately forcing MacLeod to end his life.

An Immortal seeks vengeance against a young man he believes raped his stepdaughter, leading to a confrontation at the boy's father's home, where MacLeod tries to mediate and uphold justice. As tensions escalate, the Immortal refuses to back down, threatening the household. MacLeod must find a way to stop the violent conflict before it spirals out of control.

An old friend of MacLeod's, Hugh Fitzcairn, arrives in town with troubling news—other Immortals have vanished under mysterious circumstances. After Fitzcairn's visit to Darius ends in tragedy with Darius's death and Fitzcairn's kidnapping, MacLeod uncovers a group of mortals hunting Immortals, even violating sacred ground to do so. He ultimately rescues Fitzcairn and confronts the group's leader, James Horton, though Horton escapes.
