Sarah, injured and being pursued by Neolutionists, goes deep into the forest in a desperate effort to rescue Cosima. At the same time, Cosima, recovering from her own experience, reconnects with Delphine, though both come to understand that the Island contains more secrets than they had expected. In another part of the story, Rachel becomes an important figure under Neolution's mysterious leader, P. T. Westmorland, while Alison and Donnie face mounting pressure in their search for Helena, further straining the already delicate relationships among the sestras.

Sarah is captured by Neolution and presented with a difficult choice by Rachel: either join the organization or risk the safety of her loved ones. Her decision creates tension in her relationship with Kira, who begins to come to terms with her own growing independence. Meanwhile, Ferdinand struggles with Rachel's new, more subtle method of controlling the clones, while Felix recruits an old friend to help protect Kira from both Neolution and Ferdinand. At the same time, Krystal discovers a crucial piece of evidence tied to Neolution's ultimate goals, prompting Sarah and Mrs. S to investigate the company's corporate ties to the cosmetics industry and leading to a confrontation with Krystal. Before Sarah can follow up on the lead, Krystal assumes control of the investigation.

When Helena is kidnapped, Sarah is forced into a desperate mission to rescue her, as the Clone Club unites to prevent Helena from giving birth under duress. With time running out, Sarah takes the lead in a high-stakes effort to save her sister.

Helena takes refuge behind enemy lines, compelling Sarah and Art to engage in a desperate struggle to protect her and lift the siege on the sestras.
